Student Health Services

Welcome to the Churchill County School District - Student Health Services Home Page.

This site has been designed to provide important information regarding Student Health Services, our policies, programs, staff, and services. The quality of learning for each child is directly related to his or her physical, mental, emotional, and social levels of health. The health staff tries to maintain and assure a high level of physical, mental, and emotional well being for each student.

Some services provided by the health staff (RNs, LPNs & Health Aides) include:

  • Student health appraisal and assessment (including handicapped students)
  • Management of student health concerns (including written protocols)
  • Screening programs (vision, hearing, scoliosis, dental)
  • Parent-teacher-counselor conferences
  • Medical referrals (dental, vision, hearing, and medical)
  • Health counseling
  • Communicable disease control
  • Health education (tobacco, drug & alcohol use, dental, nutrition, etc.)
  • School hygiene and safety appraisal 
  • Child abuse assessment and referral
  • Education for school staff on health related services & topics
  • Health consultation
  • Enforcement of Health Services policies (Concussion Policy/Protocols per NIAA & NRS)
  • Immunization auditing

In order for your child to be safe and healthy in school, health concerns & diagnoses, including, but not limited to the following, must be brought to the attention of the health staff:

  • Allergies (mild to severe (anaphylaxis))
  • Asthma or Reactive Airway Disease (RAD)
  • Autism Spectrum Disorder (ASD)
  • Behavior/Mood Disorders
  • Blood Disorders
  • Cancer
  • Depression
  • Diabetes

Medical Plan of Care for Special Diets

If your child(ren) has/have any food allergies or needs to be on a special diet, this form MUST be filled out and returned to the health staff at your child(ren's) school. You can download the form below.

Please print off the appropriate form,  take it to your child's doctor to get it filled out & signed, and then turn the completed form in to the health staff at your child's school. These forms must be completed EVERY SCHOOL YEAR!

Request for Medication During School Day

Please read Regulation 5150.0 for Churchill County School District's procedure for the administration and recording of medication. 

Immunization Requirements 

All 6th graders must receive the MCV4 (meningococcal) immunization and the Tdap immunization before the start of 7th grade on August 12, 2024. CLICK HERE for a letter from our Chief School Nurse. 

Based on a 2020 decision by the Nevada legislature, students entering 12th grade in the Churchill County School District will be required to show proof of immunization against Meningococcal Disease (MCV4), in addition to the previously required MCV4 immunization that your child received before 7th grade enrollment.
